5.    Current price list on file (within 6 months of date of death)
6.    Upgrade/Downgrade form – signed by contract holder’s representative (if applicable)

(Any additional forms will be sent for signature prior to benefit payment.) 

A Certified W-9 (if not already retained on file with IHLIC and verified with the IRS)
is required before a payment can be made.

6. Next of Kin Release, Subrogation and Assignment (revised July 1, 2011);
7. Upgrade/Downgrade form - signed by contract holder’s representative (if applicable)
